Sometimes your team may better at gathering information from the specialist at the hospital as from consultation with patient which is good. They may be better at expressing patient concerns than the patient. PS Answer to 8 not your teams fault. Hospital poor communication.

I have been caring for my mother for the last five weeks since her discharge from Vellindre. I live a mile away and husband had a stroke fourteen years ago but have had to stay with mam and just pope to see my husband when I could. The support I received from St. David and the district nurse was excellent especially the night sisters supplied by St. David. It was draining and emotional but with all the excellent care I was able to keep mam at home until she passed away peacefully.

There has been a complication from the time of first referral due to a re-admission to hospital complicated by the fact that the second hospital is in a different health authority: A lack of communication. Also the fact that the hospice care team can not be resumed prior to discharge from hospital means it is difficult top be confident that there is a robust and workable plan in place upon discharge from hospital. (Although I have been informed to contact the team as soon as I have a discharge date so that this can be put in place. Multi agency working would speed things up and ensure more timely.

After being diagnosed with cancer in October, I have found that St. David's Hospice care have been really helpful in every way. I know that I can telephone them at any time if need be.

Once I contacted St. David's in Newport one of the team came to visit me at home to discuss my condition, needs and requirements. They listened carefully and then offered practical advice and help to secure benefits I am entitled to as a cancer patient. This was arranged very swiftly and gave me peace of mind. The team continued to monitor my progress whilst undergoing treatment and I am thoroughly reassured by their professionalism.
