I am writing to convey my frustration, concern and disappiontment for the poor level of care/treatment received by a family member who is elder, vulnerable, frail living with multi complex diagnosis. Their file is flagged for face to face appointments only but attempts to make face to face appointment's have been rotinely declined. They are not able to advocate for themselves due to age, hearing & language issues which is why I try to help. I too have found the staff and care substandard and a life/health risk. Their care has been overlooked and this is directly contributing to worsening multi level chronic conditions complications.

Suggested improvements
I would greatly appreciate the ability to address these matters of great concern with care directly with Dr. Warden and the staff's supervisor. A patient should not feel 'they don't care about me' and as a family member I am appalled more proactive care is not provided. Staff routinely complain which is highly unprofessional. I regret the matter has reached this point.
