We received excellent care and information from the St Luke's staff prior to discharge at Derriford hospital and after discharge in Lelant Nursing Home. After discharge the staff were particularly mindful of my mother's frail condition and involve me (her son) in all of the decisions regarding her care and ensured that the necessary medications to relieve pain and ease the process, were in place. Staff made sure that we had all necessary contact details to get in contact with St Luke's staff. The St Luke's staff also made sure they were in contact with my mother's COPD community practice nurse and between us we liaised on my mother's on going care.
