The care and support received was very good. We as a family were very satisfied. However, question 3, I have marked as 3, as we never saw a care plan and my husbands care and treatment plans were not discussed,

Fully explain the care and treatment plans with patients and families as writted in the care plan, including patients preferences, likes and dislikes.
