Most of the questions are completely unsuitable for any patient and I usually decide it is not for me to recommend treatment or caring to others - which is like I am doing now. I appreciate your concern obviously, but I am not answering the questionnaire - Sorry! I have medics in the family but do not reply to any of this forms.
