Cardiac outpatient appointment for May 2017 cancelled by yourselves and rebooked for the 12 July at 10:10. On turning up on time I was told we cannot find your notes. After much coming and going I was informed my notes were languishing at Lytham Hospital. Staff promised to have the notes sent up - after 45mins I gave up and rebooked. Constructive criticism - where do I start? Staff under pressure, receptionist (on her own) dealing with a myriad of concerns from patients and consultants - but no notes. Not rocket science is it - patient and notes go together - similar to bread and butter - without both the taste and experience is awful.
