Initially there were reservations regarding key workers aid how literature was presented but not explained that it may not always be applicable to our situation. Both of us distressed to read in last year of life etc! As time has gone on any help, inquiry etc has been acted upon. Breathing clinic team at 'the hospice' - faultless. Very supportive in all aspects. This form was given on initial introduction to key workers several weeks ago it wasn't completed due to husband being very poorly and hospitalized 3 times.
