My husband passed away on 1st June 2017. I found this survey in the drawer and felt I wanted to complete it. He had his terminal diagnosis as 15/12/16 and we were introduced to the team on 29/12/16. Fortunately, the patient was in good health and the nurses were in contact regularly until they were needed during the last days of his life. I cannot express my gratitude for the nurses who came on Monday 29th May, Tuesday 30th and Wednesday 31st. Louise Taylor was wonderful and I will be eternally grateful to you all. Thank you.
