I do not feel the care and support could be improved.
Chelle was extremely professional.
Sympathetic and understanding to both my husband with MND and myself.
She give us decisions about services available and a helpline to come out which I fell is invaluable should I need it.

Outstanding care and compassion with all staff member on every level - treated with upmost respect and dignity.
We could not of asked for anything more in this last week for my dad for his final days.

Ms Karen Burdett (clinical nurse specialist) attended at my home address promptly today.
The fact that she received a number 5 (or highest performance rating) in these boxes indicates my assessment of her performance.
NB - My apologies. It is hard to complete a form when your wife is adding comments over your shoulder. I would love to see Karen again at our next appointment as arranged. If you want me to complete another form please send one out.

The nurse, Sarah, was a really lovely person and obviously had my best interests at heart. She was very helpful and answered all of my questions which gave me a great feeling of comfort.
