Good that nurse visited home and could therefore see living conditions and make suggestions as to ho adaptations could be made to improve quality of life. Home visits also mean no need to arrange transport to clinic and therefore reduces anxiety around attending appointments and specially when relying on carers to accompany you and when travelling can significantly increase pain levels, nurse provides a source of expertise and liaison between agencies which is a minefield for the patient to negotiate. Improvements necessary to educate all involved in patient care i.e GPs, district nurses and to include admin teams as well. Educate GPs to be proactive and update records immediately following palliative care consultations to enable collect medications to be issued and reduce anxiety for the patient who needs reassurance pain can be controlled as necessary.
