I called in for some help as my mother was going through a particularly bad time and the help was prompt, initially by way of telephone consultation, then by way of a personal visit by Angela Wong (CPN). Mum was prescribed some medication that helped her to settle down and get back into a normal sleep pattern, after that the communication seemed to stop except a brief email to see if mum would be discharged.

Thank you for taking the time to feedback your experiences of the service. We are pleased that you felt the help provided was prompt and that it was attended to by way of a home visit and appropriate treatment provided which improved your mums symptoms. It is vital that we respond to urgent clinical need for patients under any pathway of care within the Older Adults Community Mental Health Team (OACMHT) These pathways include Memory Monitoring Clinic (MMC) , Medic led clinics or patients who are Care Coordinated by a healthcare professional. When a Care Co-ordinated patient is admitted to hospital, we aim to maintain contact to ensure appropriate discharge planning can be facilliatted however, less contact may occur by community mental health services given the environment the patient is in. You may contact the team at any time between the hours of 9-5, mon-fri should you require advice or support.
