Exceptional care given by exceptional people. Great nurse Liason service showed how a service should be. Would have preferred to see them on 1st visit to ward but we weren't there then. But they did a great job. Referral passed on from A and E, well done - smooth transition. This is important - that teams pass patient along well. Very very thankful for the professional, kindness.
