My reason for submitting the answers I have submitted is because, they indicate as accurately as possible, I feel about those questions. I was not examined, therefore I was unable to answer the last question, it states "If an examination was required, were you asked to give consent to the nurse?" If one answered 'not at all'. That would be misleading. It would be better if it read something on the lines of; Was an examination required? (a) yes (b) no
