The care and coordination of services, especially when my mum finally was diagnosed at 'end of life'. Also she was reluctant, at first, to go to the social, but absolutely loved it when she did.
One thing that could be improved is information that is centralised - who does what:-
Community nurses.
Hospital.
GP.
Hospice care.
Marie Curie.
Devon Doctors.
2 times HR responders.
UCR.
999.
Along with numbers and operational times.
I suggest a chat - to the layman, it is bewildering.
