I brought my 10 month old son to be seen after previous been hospitalised on IV antibiotics the previous week. Appointents we’re running 2 hours over, which I understand and appreciate it was a busy time. I am really not happy that false information was recorded on his medical history and was only changed as I questioned it. When taking his initial observations, the nurse/HCA only took his temperature, but falsely recorded values for heart rate, respiratory rate, oxygen saturation etc. I noticed this in the consult room when with the doctor. I made the doctor aware, and he repeated them. They were all ok, but the previous week his O2 sats were 88%. So this could have been a very different story. The doctors are working off the values inputted.
