MENTAL HEALTH CONDITION Please see medical records which show that James was diagnosed in 2019 with an it and depression with some psychotic symptoms. Since early 2020 he has been unable to work and has significant problems with socialising He has had a sleep disorder for many years He can’t answer his phone or reply to texts He has cut off friends and family, and relates now only to his mother and father. His anxiety means he can’t organise himself or commit to any particular action At times he feels rage and will struggle to contain this ASTHMA James had asthma as a child and teenager including one hospitalisation. No incidents in recent times but James still avoids dusty and confined spaces

Suggested improvements
It was not ideal having four people in a small and almost windowless room for James’ first meeting with you. Also concerned that James’ medical records for the second meeting - where you saw James alone - include the words “James denies any problems with sleeping or eating” James has a long standing sleep disorder, which he reported to Marcin Prochnicki on 26th May 2022, as stated in Mr Prochnicki’s records of that consultation. He quotes James saying:Very poor sleep… never had a good sleep. Appears to be linked to inability to switch off…” Also, James does have some issues with when and what he eats, and was receiving therapy for this from Health and Well-being Coach Sophie Cooper at his GP surgery from 25th March 2022 to 17th June 2022. So it seems unlikely that James would mean to say that he had no problems with sleeping or eating when he saw you that second time. It’s possible James could have misspoken as it was a session in which you were reviewing childhood trauma with him. If it’s possible for you to review your notes as to the words James used about sleeping or eating, or the context in which they were said, that would be helpful
