Despite regular attempts to engage with staff about discharge planning the first active contact received was a week after admission on the day of discharge to say where I was going. The arrangements had been put in place by daughter in law with no help from the ward other than booking transport. When visited on several occasions I was found to have been incontinent under my chair. No attempt at active rehab appeared to have been made
