We completed the travel questionnaire over eight weeks ago for our upcoming trip. Eventually my wife heard from old town surgery who arranged an appointment with the nurse, when she asked about my appointment she was told my form had not been assessed yet even though they were both put in at the same time and we are going to the same places at the same time. Eventually we got appointments to see the nurse, but these were cancelled and rescheduled twice, until yesterday when we arrived at the surgery for the appointment and was told it was cancelled again!!! And this time I had paid for the car park we were told by the receptionist someone had tried to call us but on checking no call had been received on either our home or mobile numbers. It is now too late to rearrange as we travel in less than 2 weeks. On the whole this is a totally unacceptable service.

Suggested improvements
Employ nurses that are not always sick. Improve job share so it does not have to be the nurse who reviewed your original form that you have to see. This was a totally inadequate service right from the start.
