Appointments should be on a first come first served basis not on who can ring in the quickest at 8 am, with time for emergencies after surgery. If I need an appointment I would like it in the next 2-3 days not in 2 weeks time, and that's only if there is one available online. Also, all the wasted appointments where people don't turn up, I would like to suggest 3 strikes and their out. This may seem harsh so maybe you could come up with some other penalty like a fine. Finally, I think some of the staff could do with training in being more patient friendly.
