staff always seem polite and friendly

The forms we are giving to help with feed back, I am happy to fill in, but they are more for the patient and not for family and friends just visiting. There is no place to N/A, I am a visitor and would have nothing to do with care planning, neither would I be given information about the care that is being received. It would look like I am marking the hospital down by the way this form has been developed.
