Kindly treatment with lots of care. It would have been helpful for one to know when it is most convenient to staff to prepare me for bed. I do understand that change over of staff and other duties make some times inconvenient. I do want to be a considerate patient.

Spoke to care home manager, she cannot believe H @ H service has been running over a year and only knew about the service in September for it was required for one of the residents. Advised how to request a visit and referral process. She really appreciates a brilliant service in place at night.

Spoke to patient's wife, she wishes hospice at home was put in place and trinity services sooner, hospice at home only visited once he passed away as Health centre kept admitting him into hospital instead of managing symptoms and giving support at home.
