As you were informed on 26/10/17 (as noted in your form) that my husband wanted to return home and wasn't eligible for ongoing NHS care, why did it take you till the beginning of December to appoint a O.T worker to help, despite many phone calls from us. Also you say a ramp at our home didn't meet blde regs according to the blue lamp rep, Martin, this was nonsense as it was an ideal site and slope for a ramp. According to the discharge form - his discharge is dated 20/9/18 so he must still be in hospital!! The wheelchair was returned.
