It was the GP who suggest contacting single point, and it took us some time to work out how each "agency" would provide their mutually exclusive support.
Similarly it was a steep learning curve to understand the relationships and responsibilities of the different agencies with regards to who prescribed medication, how they were administered for repeat prescriptions, discontinuing medications, and how to prevent repeats of prescribed medication that were not currently being used. My partner was discharged 2 days before Christmas fitted with a syringe driver and taking oral meds that were not regularly available through local pharmacies. She has now passed away. I would recommend that any improvements that can be made to GP agency GP communication would be most welcome - particularly for the patients and their carers.
