Medically no problem but getting forms to claim for allowances is taking a long time, i've offered to go into age concern and sort it out myself.

I have only been introduced with one home visit. At this moment in time I am attending hospital appointments for treatment as and when necessray. The St. David's team that have been involved with me has been very good. They have put me in the driving seat behind my care and then pointing me in the right direction as required . In due course, I a sure that I will have more reliance on them and hope that when the time comes all will be in place. Todate, I am very happy with the care received by my St. David\s team.

I live on my own in an upstairs Council flat. My daughter and my sister always come round and I rely on them to take me shopping and little trips out. But everybody I look forward to my Monday's meeting at St David's Hospice where I meet people, and enjoy being spoilt by their careers. The meals are first class. We need more places like that, I'm lucky, it gives me something to look forward to. Thank you. Mrs J Campion.

Thank you for sending us his communication.However we have only had two visits/ contacts with the palliative nurse and therfore do not feel able tocomplete the questionnaire.It is too soon. So far our experiences would ask when is it right to (1) talk about D M R declaration and (2) where does one want to spend the last for weeks/ days of your life and is it right to discuss such improve.

I have only recently been referred to fr David`s following two weds in hospital. I have had help to apply for attendance allowance and a blue badge. I have also been referred for reflexogy which I am enjoying really answer all the questions but so far I have been impressed by the professionalism and the staff that I have met.
