I don't think you can improve.
Tracy who comes to us fulfills all that we require so far and she listens to everything we say.
Hollie has called out to see our dad, she’s always a call away, but dad is doing ok for now.
Not enough time since hospitalisation to answer this.
Within 2 days of my G.P. referral I had a telephone call from St. David’s Foundation + the following day a avisit from a nurse for assessment. Very soon a visit from an Occupational Therapist. The O.T. suggested a stair lift would be recommended. About a week later this was done. Meanwhile I had a visit from a man who came to fit grab rails in downstairs cloakroom and en-suite upstairs. Follow up phone calls and visits have been made and a visit from a financial adviser. I am delighted and relieved to have such support.
I have had great support from the St. Davids Hospice nurse, Day hospital and Dr Glesoon. They have been a great support.
My wife, Mary Theresa Kimf, is mentally and physically incapable of completing this form. However, the only care provided by your organisation has been on certain Sunday mornings, for approximately 2 hours, to enable me to go to church. So far, the carer that has attended has been most satisfactory, but I cannot complete the items on the section one.

Fantastic support from Claire – nurse Catherine, Jeanette – social workers. Don’t know how we would have go through each day so far without the teams support. I feel fully confident of the service we’ll receive in the future at such difficult times. Thank you all
