I am responding on behalf of my husband, I was very saddened to receive this two weeks after my husband passed away. My concerns with the service provided. 1. Failure to have in place an end of life plan. 2. Unable to speak to a nurse via the out of hours emergency service and days before his death. 3. Nursing staff service not to assess how poorly my husband was during last few weeks. I feel he was let down by the service - too little too late.
