When first referred to Palliative care team-they kept in touch weekly by phone between units. After 6 mth this stopped and it felt like we had had our alloted session
However soon as ever I (The cover) contacted the team because the patient was 'in pain'- the response was fantastic an appt was made to wait at home the next day. Patient needed lots of TLC to be encouraged to 'when a bed available' to be admitted to Rowcroft to try + improve pain control etc. Patient is very frightened but Lorrie was very reassuing - i.e that. p. patient when he can go home at anytime.
Also TLC for vare on way out was very encouragin as patient + carer very low in spirit
