For all the reasons listed overleaf

Maybe review your procedures on admission of course you need as much information as possible but it was very overwhelming to have to fill in 3 separate forms immmediatly on arrival when I was exhausted and stressed. John had a 8 minute seizure while we waited for the ambulance so a bit more flexibility on timing of form completion would have been welcome
