Writing this from a family member point of view as I had to accompany the patient. Talked to dept via phone prior to pre assessment as we had concerns regarding aftercare due to poor memory and slight dementia of patient who is also under the memory team. I was told this would be discussed with options of aftercare and respite. At the pre assessment the nurse although very good with the questioning of patient said she could not discuss any aftercare. The patient did say she understood what was required. I knew this would probably be forgotten and it was. After the cataract was done I went in with patient for the discharge procedure. The patient became distressed and argued that she was never told about what she had to do with the eye drops. Unfortunately the discharge nurse due to the lateness of the operation and having to see other patients, did not have time to talk it through for patient to understand. I got patient home and settled and went through what she had to do. I watched her attempt to put eye drops in and she could not do it. We contacted her doctor who put a request in for district nurse help for drop administration. 5 days later I have tried to get once or twice a day. The district nurse has been once and shown her the best way of applying drops.To date my wife has tested positive for COVID and I am not going to be able to apply drops.

Suggested improvements
Unfortunately there was no allowance and consideration for a vulnerable person living on her own with medical issues. After a phone call discussion with the ophthalmology dept that indicated that all would be covered at the pre assessment, I was took a back when the nurse said that she could not help with anything. The patient did not bring prescription information and did not know what she was taking , I was surprised the ophthalmology had no computer links to this or any medical history. Thus I feel that full medical history and a plan for elderly people who live on there own is required.
