This is about several encounters. Also to avoid patients having to learn it themselves & get in disputes with staff & create an atmosphere of litigation politics. I have AF that has led to heart failure -overstretched chambers & valve leakage that cannot be cured. This is due zero advice till it’s too late & even then I had to find out myself (how to monitor & limit & report medication success or failure). If the discharge notes computer generated attached care plan advise to patients for self management whenever AF is entered (or other conditions that benefit from similar advice) - then it would cure lots of risk factors & general lack of expertise by being a regular reminder to both patient & staff & Locum doctors etc. of such critical risk advise.

Suggested improvements
Add computer generated action plans to patient Discharge Summary whenever patient self manage advice & ongoing risk factors apply. eg in this case Heart rate that patient is completely & continually unaware of for AF could well be maintaining itself at levels that risk stretching the heart chambers & causing irreversible damage. Even when on medication it still needs checking (in my case it wasn’t working for months & I had no idea so rather than waiting months for the next cardiology check, or even to see them in the first place; it is essential to check it & report any sustained high rates to prevent further damage.
