She help and support of Jodie and Karen made a huge difference to my outlook on my life.
With their guidance my suicidal thoughts were stopped and I learnt to think positively about life.
I could not have gutter through without their help.

- Opportunities to practice exercises set in a small group situation.Handouts to take away and study no need to read through as a group the hand outs given at the end of the session. Participants could as an alternative, be asked to highlight the areas of new knowledge/understanding pointed up in the info. Sheets provided.

I did not find filling in this form easy. I am the carer, my husband with dementia would not have been able to fill this in. He would have ticked just any box I would recommend you have 2 forms.
-One specifically for the carer.
