Suggested improvements
I would not like my comments to reflect badly on any staff who are clearly time and resource pressured. The service seems to be seriously underfunded and near breaking point. Staff are doing the best they can in difficult circumstances. Nevertheless there seems to be little time for reading up on notes which can result in the situation where you have to repeat the same distressing story multiple times. The whole setup with the different teams seems unnecessarily complex - the overlaps are always going to leave space for people to fall between the gaps. There was a need for improved handover from the crisis house - I was not stable or ready to leave. I left in some distress without any kind of parting interview. If hospitalisation is to be avoided then perhaps an extended stay in the crisis house should be an option (in my case there was space at the time). I requested hospitalisation but because I did not seem to have an immediate plan for suicide this was not considered appropriate. There is an obvious fault with this logic: if I am at the point of having a plan then it is too late as I will not want to share it. If an extended stay in the crisis house had been possible or if I had been placed in hospital I would have been saved two suicide attempts, one after the other in just a couple of days. Dr [name removed] was fantastic - he reviewed my situation and medication and gave me the time I needed. However it had been well over a year since my last appointment with a psychiatrist and I had been requesting a review for some time. It is unfortunate that a compete crisis seems to be a prerequisite for being seen!
