Home from hospital visits. Excellent and reassuring for both patient and carers/family. The respect and care all staff showed was fantastic. The only shall problem we encountered was when patient needed a driver and a bank nurse came who had no work phone and really struggled to get through to district nurses. The end of life 3 hour sessions-fantastic but maybe the home from hospital could've been made shorter if not needed? Overall-you enabled both family and patient to have care, dignity and as good as end of life care could be. Thanks.

Thank you for your lovely feedback regarding the care provided by the team. The home from hospital provision is assessed on an individual basis and the Hospice at Home team will discuss with patients and families and provide less shifts if this is required. All the bank nurses for Hospice at Home have access to a phone, potentially the difficulty was in getting hold of a District Nurse at that time. We are working with the District Nursing team to improve the ability to contact the team when needed.
