Suggested improvements
Involve patients and their families in decision making at the outset. Don’t wait until you have made the decision before involving them, leaving them no space to query or contest the decision.
Let patients and their family know what services/therapies are available. Don’t withhold this information from them. They should know this from when they first access the service.
Don’t make statements like, ‘you can access therapy when you are stable’, then don’t give a definition of what stable looks like.
Treat patients respectfully, not just when their family are with them but also when they are on their own.

The only good thing was the transitions social workers assessments of my children for their care act assessments.

You could listen. You could work collaboratively. You could explain how each part of the service works. You could explain how communication should take place. You should explain that staff turnover is so high that your cared for will see someone different every time. You could read the notes on a patient before seeing them. You could read the right notes-my sons first psychiatrist was reading the notes of another young man in our meeting and not those of my son.
I have made contact via telephone and email in excess of 20 times since Oct asking for my son to have a review. I have had to call the crisis team out of hours due to my son experiencing new and scary symptoms. Even they said he needed a review as a matter of urgency. Management said they would inform my son and/or I of his review apt by email or phone. They did not do so until the day of the apt. During that apt, my son who just 6 weeks ago was hallucinating, hearing and seeing things, disassociating and suicidal was told after just 25 mins with a new psychiatrist that he was being discharged back to his GP. 25 minutes! That’s not a review. That’s being brushed off. It’s appalling. Neither he, nor I as his carer, had this explained properly to us. Neither of it knows what it means. I do not know what avenues of support are available to him or to me.
