It was a good way to find out information about dementia and where to get advice if and when the condition worsens.
I found the course really helpful and enjoyable.
Penny was respectful and friendly and I felt comfortable with the whole assessment. The appointment letter did not refer to the questionnaire enclosed. I thought it was for when they came so subsequently my daughter was not informed when the date was changed. Also, several of the questions caused me anxiety until my daughter looked upon the intern what it was (eg section 117??)

I apologise you found the questionaire anxiety provoking. The qustions you refer to are part of the information gathering process that we use to undertake an assessment and we would always be happy to discuss and explore any questions or concerns you may have

Everything was just perfect. No problems.
Angela very considerate and patient with service user, discussed plan going forward in depth to all parties (service user, her husband and myself (manager).
I cannot think of any improvements, very happy with service.
