A midwife had given me medication and did not write it down in my medical notes this caused a discrepancy when I was taken down for an emergency C-section. As this medication she injected me with was a blood thinner. I also felt like I was not performed about most of my care plan.

Inform patients as to what is happening with their care on a day today basis, also to include them more in their care so they feel less anxious with whats happening with them and to not mess with the patients mental health by promising something you cannot promise.
