We are pleased that you felt the help provided was prompt and that it was attended to by way of a home visit and appropriate treatment provided which improved your mums symptoms. It is vital that we respond to urgent clinical need for patients under any pathway of care within the Older Adults Community Mental Health Team (OACMHT) These pathways include Memory Monitoring Clinic (MMC) , Medic led clinics or patients who are Care Coordinated by a healthcare professional. When a Care Co-ordinated patient is admitted to hospital, we aim to maintain contact to ensure appropriate discharge planning can be facilliatted however, less contact may occur by community mental health services given the environment the patient is in. You may contact the team at any time between the hours of 9-5, mon-fri should you require advice or support.
