Penny was respectful and friendly and I felt comfortable with the whole assessment. The appointment letter did not refer to the questionnaire enclosed. I thought it was for when they came so subsequently my daughter was not informed when the date was changed. Also, several of the questions caused me anxiety until my daughter looked upon the intern what it was (eg section 117??)

The appointment letters are being reviewed currently, along with our leaflet packs, to ensure they capture all the information that is required. I apologise you found the questionaire anxiety provoking. The qustions you refer to are part of the information gathering process that we use to undertake an assessment and we would always be happy to discuss and explore any questions or concerns you may have
